Your gifts to the Lakewood Health System Foundation will help fulfill healthcare needs in our hospital and in the communities we serve -- enriching lives every day.

The Lakewood Health System Foundation was established by the Lakewood Health System Board of Directors in 1994. The Foundation is in the hands of the Endowment Board which consists of eight volunteer community representatives, two Lakewood physicians, the president of the governing board, and the president of Lakewood Health System.

The well-being of our communities is our focus. With your support, Lakewood Health System is able to make a difference in the lives of our neighbors - providing the best possible care today, and into the future. Through our hospital, clinics, senior housing, and Care Center along with our programs such as Home Care, Hospice, Palliative Care, support groups, and educational opportunities, Lakewood Health System Foundation is able to impact thousands of lives. We work closely with volunteers, physicians, staff, community members, and neighbors to keep our products and services current and cost-effective, and our communities happy and healthy.

With your help, we pledge to continue to make a difference in the lives of our patients, their families, and loved ones. Thank you for your support!

The Foundation holds a number of events throughout the year to encourage charitable gifts in support of community health-related projects. Events include:

Dinner Auction
The Dinner Auction is an annual event to raise needed funds for the foundation and the great projects it supports. Each year, the Foundation grants dollars for health related projects/programs to businesses, schools, and organizations and our hospital needs.

Golf Classic
The annual Foundation Golf Classic is held at the Vintage Golf Course in Staples every August. Teams play 18 holes, with an afternoon tee time. All funds raised go towards healthcare needs in our hospital and our communities we serve.

Seasons of Lights
Presented by the Lakewood Health System Foundation and Hospice Program, the lighting ceremony is a meaningful way for families and friends to pay tribute to the memory of a special loved one or honor someone living by purchasing a light for the memorial tree. Contributions benefit our Hospice Program. This event is held the first week in December each year. It is held in two separate locations because our Hospice Program reaches out to such a wide service area.

The theme of the Lakewood Health System Foundation’s 24th Annual Dinner Auction is “Suite Fiesta.” This year’s dinner auction will take place on Friday, April 7, 2017, and proceeds raised will help fund the renovation of two respite/hospice rooms at Lakewood Pines senior assisted living facility. The Foundation has set a goal to raise $40,000 for this project.

These “Suite Time Away” rooms will provide care for loved ones, which will offer a safe, comfortable place for the loved ones of caregivers in our community. This will in turn give those caregivers an opportunity to rest and take a break without worry.

By filling out the form, you can share with us the great care and service you've experienced.

And if you’d like to show your appreciation to those who gave you care, a donation in their honor is a great way to say ‘thank you’. To donate, click on the link at the bottom of the page. (Please click the 'Grateful Patient' bubble under the 'Designation' header on the form.)
