The Lakewood Health System Foundation was established by the Lakewood Health System Board of Directors in 1994. The Foundation is in the hands of the Endowment Board which consists of eight volunteer community representatives, two Lakewood physicians, the president of the governing board, and the president of Lakewood Health System.
The well-being of our communities is our focus. With your support, Lakewood Health System is able to make a difference in the lives of our neighbors - providing the best possible care today, and into the future. Through our hospital, clinics, senior housing, and Care Center along with our programs such as Home Care, Hospice, Palliative Care, support groups, and educational opportunities, Lakewood Health System Foundation is able to impact thousands of lives. We work closely with volunteers, physicians, staff, community members, and neighbors to keep our products and services current and cost-effective, and our communities happy and healthy.

The Foundation holds a number of events throughout the year to encourage charitable gifts in support of community health-related projects. Events include:
The Dinner Auction is an annual event to raise needed funds for the foundation and the great projects it supports. Each year, the Foundation grants dollars for health related projects/programs to businesses, schools, and organizations and our hospital needs.
The annual Foundation Golf Classic is held at the Vintage Golf Course in Staples every August. Teams play 18 holes, with an afternoon tee time. All funds raised go towards healthcare needs in our hospital and our communities we serve.
Seasons of Lights
Presented by the Lakewood Health System Foundation and Hospice Program, the lighting ceremony is a meaningful way for families and friends to pay tribute to the memory of a special loved one or honor someone living by purchasing a light for the memorial tree. Contributions benefit our Hospice Program. This event is held the first week in December each year. It is held in two separate locations because our Hospice Program reaches out to such a wide service area.
Lakewood Health System is an integral part of our community and economic development, and the Lakewood Foundation exists to ensure that our communities continue to thrive and grow. Gifts from supporters like you enable us to provide vital services far beyond what we are reimbursed for, and help us fulfill our mission of improving the health of all members of our communities we serve.

Planned Giving - When planning a future gift, it’s sometimes difficult to determine what size donation will make sense. Including a percentage of your estate ensures that your gift will remain proportionate no matter how your estate’s value fluctuates over the years.
Living Will - It's pretty simple to add a few sentences to your will or trust . The specific bequest language for Lakewood Foundation is: “I (name), of (city, state, zip), give, devise and bequeath to Lakewood Health System Foundation (written amount or % of estate or description of property) for its unrestricted use and purpose”
Stock, Real Estate or Personal Property - You can give to the Foundation by transferring stocks, assets or investments, or by donating personal property and /or real estate.

23rd ANNUAL LAKEWOOD HEALTH SYSTEM FOUNDATION GOLF CLASSIC - AUGUST 25TH, 2016
The Lakewood Health System Foundation will hold its 23rd Annual Foundation Golf Classic, Thursday, August 25th at The Vintage at Staples (27923 McGivern Dr., Staples). Proceeds from the event will go toward the funding of community health, and children’s programs.
Co-sponsored by Nyhus Chevrolet-Buick and First International Bank & Trust of Staples, the four-person golf scramble offers participants a 12:30 p.m. tee time, with registration, lunch and program starting at 11 a.m.
Lunch and appetizers will be provided and participants will also be eligible to receive various prizes and awards. All proceeds from the tournament will go towards Lakewood’s children’s health and wellness programs.
Donation opportunities include:
• $400 per team—includes four playing spots, two golf carts, food and prizes.
• $250 hole sponsorship—includes a sign at the sponsored hole, a listing in the program, and the opportunity for a sponsoring business to provide give-a-ways at the hole.
The Foundation Golf Classic has room for 36 teams. Early registration is appreciated. For more information on this event, contact Megan at 218-894-8508. Registration ends Wednesday, August 17th.
We hear from patients and their families every day about the exceptional, compassionate care they’ve received from providers and nurses, nutrition and housekeeping staff, and various other staff members at Lakewood Health System.
Our Grateful Patient program provides a way for patients to publicly thank those who’ve given them care, and, if they so choose, financially support the programs and services at Lakewood with a monetary donation.
