Lakewood's Service Excellence Program
This program recognizes our employees for outstanding performance in the delivery of service to patients, residents and visitors, and how they work as a team with other employees. 

Our customers are welcome - and encouraged - to nominate staff for outstanding service. Fill out and submit the electronic form below, or click on the "Documents" icon on the right to nominate an employee for a S.T.A.R. (Service & Teamwork Award Recognition) award. Five employees are selected each month to receive a S.T.A.R. award, from the nominations received. Employees are selected for recognition based on the following criteria:

  • Provides service above and beyond expectations.
  • Completes job with enthusiasm.
  • Responsive to ideas, suggestions and concerns of patients, residents, visitors and fellow employees.
  • Willing to help others and go beyond job description.
  • Identifies and follows up on problems to ensure they get resolved.
  • Portrays a positive image of the facility in and out of work.
  • Willing to make decisions and meet patients, residents and visitors needs with commitment and urgency.
