Social Services Social Services provide a variety of services for hospital patients. They work as an advocate for patients and are available to meet with patients and their families to discuss any questions or concerns they may have. They provide discharge planning for patients in need of post-hospital services. Examples of discharge planning needs may include: home health care services, meal assistance, financial assistance, home medical equipment, etc.Swing beds and transitional beds are also a service available at Lakewood Health System. Social Services assist in the assessments and admissions of these patients. Swing beds and transitional beds are utilized for patients needing extended care services that meet appropriate insurance criteria.
