Care Van

NON-EMERGENCY MEDICAL TRANSPORTATION

Lakewood Health System’s state approved, specialized transportation service for non-emergency medical services.

Care Van takes you door-to-door and our drivers will help you get both into and out of your home and medical destination.

Our drivers are courteous, friendly, helpful and prepared. They are all equipped with cell phones, so that they can be reached or place a call in case of an emergency.

Whatever your needs are, we’ll do everything we can to help-including helping you register for your appointment. AND, caregivers are always welcome to ride along absolutely free of charge

WHERE YOU NEED TO GO

We’ll take you just about anywhere you need to go-across town or across the state.

If your appointment is at one of the Lakewood Health System Clinics in Eagle Bend, Browerville, Motley, Pillager or Staples... we’ll see that you get there!

Eye appointments, dental appointments, chiropractic care, therapy or counseling sessions... we’ll see that you get there!

If you have a referral to an out-of-town site like St. Cloud, Fargo, Little Falls, Brainerd, the Twin Cities or a VA hospital... we’ll see that you get there! Tell us when, tell us where and... we’ll see that you get there!

You are also welcome to leave a message at this same number 24-hours-a-day, seven-days-a-week, and our staff will contact you as soon as possible. Naturally we prefer as much notice as possible, but we will do everything we can to accommodate your requests.

Your ride may be covered by your insurance health plan. Payment can be made on the date of service or you can arrange billing through our office. Call our office for an estimate of the charge for your specific transportation needs.

$5.00 (round-trip)
$3.00 (one-way)
Payment due at time of service

Rates vary for persons who are wheelchair-bound. Call our office for approximate rates and health plan coverages. Caregivers are welcome to ride along for free. *Call for health plan coverage for persons needing assistance.
