2. The PDF version will allow you to open the file in Adobe Acrobat Reader and type in your information. However, you will not be able to save the file to your computer or submit it via email. PERFORMANCE STANDARDS DOCUMENT

The Standards of Behavioral Excellence form must be signed and dated before submitting an LHS Application Form. Please include Page 6 of the document with your application. Any application / resume submitted without the Standards of Behavioral Excellence signature page will not be considered. [click here] to download and sign the form.
