At Lakewood Health System, outstanding primary care is truly the heart of our practice. Committed to providing care for the entire family, Lakewood brings the perfect blend of personal attention and hands on care, with all of the convenience and modern technology you’ve come to expect from a healthcare system. Whether seeing you in person or through a virtual visit, Lakewood continues to do all it can to deliver outstanding care. Online scheduling and provider reviews, virtual visits, Click Care, and five convenient clinic locations bring you the care you need whenever and wherever you need it.
The care, support and education you receive from your primary care provider and their team includes everything from preventative screenings and exams for all ages, care of everyday health problems and issues, as well as the care and coordination needed for any specialty concerns.
In order to help our patients achieve their best health and well-being, our current model of care, otherwise known as “care teams”, have been put into place to enhance the care patients already receive. These care teams consist of physicians, physician assistants/nurse practitioners (advanced practice clinicians), nursing and clinical support staff. These care teams are broken down based on the clinics in which the providers practice.
Finding the Primary Care Provider (PCP) that’s Right for You
Finding the right provider for you or your family depends on your age, goals, and needs. At Lakewood, you can choose a:
- Family medicine doctor – cares for all ages
- Family medicine doctor with obstetrics (OB) – cares for all ages, provides pregnancy care, and delivers babies
- Pediatrician – cares for babies, children, and adolescents
You also may see an advanced practitioner, such as:
- Physician assistant (PA)
- Nurse practitioner (NP)
Your primary care provider may refer you to a specialist to assist in your diagnosis and treatment. Some insurance providers require a referral in order to cover the cost of certain services.
