Lakewood Care Center
Lakewood Care Center is a 100-bed, Medicare certified skilled long-term care facility recognized by Nursing Home Compare as one of the best nursing homes in Minnesota based on survey results involving nursing staff, health inspections and quality measures. Our philosophy focuses on providing a safe, pleasant, home-like environment for our residents. Our services are designed to improve living skills, and we see many of our residents actually return to their own homes.
At the center of our services exists a pledge to treat every person with the respect and dignity they deserve, and to focus on improving our residents’ quality of life, by delivering the highest quality of care.

Lakewood Care Center information
Our Activities department is located in the Care Center on our Senior Campus in Staples. The Activities department is designed to:
- Meet the individual residents’ social, physical, mental and spiritual needs.
- Promote and encourage the resident to function at the highest level of independence possible.
- Reassess and adapt activities to suit the needs, capabilities and interests of each resident.
Group activities include:
- Cards, dominos & games
- Fancy Fingers
- Discussion groups
- Men and ladies’ groups
- Church services
- Music
- Exercise
- Reading groups/book club
- Monthly cappucino time
- Social hour
- Pet time
- Adapted sports
- Bingo

Activities designed especially for residents with special needs include:
- Memory Care Unit
- Neighbors—a motivation group
- Sensory stimulation—a group for residents who require stimulation of all senses.
- One to one visits—the staff will visit individual residents in their own room if they choose not to attend regular activity functions.

We know many people are concerned and wondering about the spread of COVID-19 and how that impacts Lakewood Health System; in particular, our care center and residents.

April 29, 2022: There have been some updates to the visitor screening process for Lakewood Care Center.
- Upon arrival, visitors will follow instructions for self-screening, put on a mask and eye protection (located in the entry area) and take their ‘ticket to visit’ (screening form) to the department you are visiting.
- The doors to enter will still be locked and visitors must use intercom to request entry after self-screening is complete.
November 17, 2021: While Lakewood Care Center is currently in outbreak status, per CMS guidance, regular visitation is once again open. Below are some things to note:
- Visitors who are positive for COVID-19 or meet quarantine criteria should and may not be allowed to visit.
- Advanced scheduling is no longer required, but we encourage you to call ahead to determine the quarantine or precaution status of your loved one prior to visiting.
- All residents and visitors, regardless of vaccination status, must be masked and physically distanced at all times.
November 11, 2021: We are experiencing an outbreak of COVID-19. To ensure the safety of our residents, staff and visitors, we are asking visitors to consider waiting on visitation until the outbreak has been contained. While we are not stopping visitation for essential or compassionate care, we are unable to allow open visitation at this time. We want to make you aware of the situation, so you can make an informed decision. Please call 218-894-8345 with questions. Thank you for your help and understanding as we work to maintain the health and safety of all!
November 1, 2021: Effective immediately, the care center is open for visitation. Visits no longer need to be scheduled ahead of time, but we do ask for compliance with MDH and CMD guidance in order to continue with this change in policy. Visitor screening and check-in, as well as visitors wearing masks and eye protection during their visit, will continue. August 24, 2021: Due to a positive COVID-19 test at our care center, we are limiting visitation until further notice, per the Minnesota Department of Health guidelines. At this time, we are only allowing compassionate care and essential caregiver visits. To schedule, please call 218-894-8345. We apologize for the inconvenience and will do our best to update this information here and on our Facebook page as soon as we’re able to resume regular visitation.
Like a warm embrace, our Memory Care Unit is a safe and comforting home for those affected by Alzheimer’s disease and other dementia. Feeling secure and “at home” is something we all want. The care and interaction from our staff, combined with a spacious, private and modern facility is aimed at providing each person the very best living environment possible.
The Memory Care Unit strives to create an environment for our residents that feels and functions like a family. Our staff is specially trained to assist those with memory loss. We take the time to create familiar routines and help our residents take pleasure in the milestones of each day.
Residents are encouraged to remain active and spirited with purposeful activity specifically designed for memory care patients. Movies with popcorn, taking the time to bake and strolling in the garden may trigger special memories for our residents.
We make sure that many of these activities and more are incorporated into daily routines as often as possible.
