Clinic Fees

Current, standard charges for the top 25 procedures over $25 at each of Lakewood’s primary care clinics. This includes: the clinic’s charge for the service, the average commercial payment, the Medicare reimbursement rate and the Medical Assistance reimbursement rate. Prices are current as of 7/1/2019 and are subject to change.
