A healthcare directive gives your healthcare providers and/or your designated healthcare agent(s) the guidance to make healthcare decisions according to your wishes. It only applies when you cannot make decisions or communicate your wishes. The document may include what treatment you may want or may not want under specific circumstances.
Social services at Lakewood Health System can assist you in filling out a healthcare directive and assuring your healthcare provider has this information in your medical records.
Lakewood Health System uses the Honoring Choices Minnesota healthcare directive, which is an initiative of the Twin Cities Medical Society. To download a directive, click the Documents link.
