Lakewood’s Health Information Management (HIM) team maintains patients’ health records. They also handle vital statistics and records (such as birth certificates) and process requests for the release of health information.
Our team members in this area gather information and process your baby’s birth certificate with the state of Minnesota. We also offer to apply for your baby’s social security card, and send birth announcements to area newspapers. Vital Statistics also works with physicians to process death certificates and cremation approvals for your loved ones.
Authorization and proxy forms for MyChart accounts can be found under the Documents tab to the right. These forms allow Lakewood to share your personal health information with you, as well as a proxy, via the MyChart platform.
Release of Information
Our team members in this area perform such tasks as obtaining patient consent, certifying medical records, and deciding what information can be released. They will be happy to handle any request for information, whether it is for personal use, transfer of care, legal, or insurance.
You can now request access to your medical records online! Click the link below and you’ll be guided through the process. Once your request is submitted and approved, you will receive your records in the method you chose (electronic or mail).
*Our online medical records request will ask for a photo of your driver’s license. We use this photo to verify your identity. You can take the photo with a webcam or smartphone. This process works best in Chrome, Safari and Firefox browsers.
*Requests for imaging records cannot be sent electronically, so you will receive them via the mail.
