We offer three convenient ways to schedule an appointment with a Lakewood provider: via phone, online through our website, or online using the MyChart patient portal.

Visit Options

The care, support and education you receive from your primary care provider and their team includes everything from preventative screenings and exams for all ages, care of everyday health problems and issues, as well as the care and coordination needed for any specialty concerns.

Patients experiencing any number of minor ailments/illnesses can be seen in our Staples Quick Care clinic for a quick, 10 minute appointment. Ailments/illnesses include:

  • Common allergies
  • Athlete’s foot
  • Bladder infection symptoms
  • Bruise/minor abrasion
  • Minor burns
  • Cold and canker sores
  • Cold symptoms/congestion
  • Cough
  • New-onset diarrhea
  • Earaches/ear infections
  • Fever
  • Flu symptoms
  • Insect bites/stings
  • Itchy eyes/styes
  • Lice
  • Nausea/vomiting
  • Pink eye
  • Poison ivy
  • Removal of stitches/sutures/staples
  • Sinus symptoms
  • Minor skin infection/rashes
  • Sore throat
  • Sprains/strains
  • Sunburn
  • Swimmer’s itch
  • Tooth pain

Patients can also be seen for symptoms consistent with COVID-19. Typical symptoms associated with COVID-19 are:

  • Cough
  • Diarrhea
  • Fatigue
  • Headache
  • Fever/chills
  • Muscle/body aches
  • Nausea or vomiting
  • New loss of taste or smell
  • Congestion or runny nose
  • Shortness of breath or difficulty breathing
  • Sore throat

Weekday Quick Care appointments must be scheduled, but Saturday visits are by walk-in at our Staples clinic from 8 a.m. to 12 p.m.

Our Staples clinic offers same day appointments with a family practice provider for non-emergent illnesses and conditions, such as:

  • Acute abdominal pain
  • Acute injury evaluation
  • Acute onset joint or back pain
  • Anxiety
  • Blood clots or leg pain
  • Broken bones
  • Cuts requiring stitches
  • Dislocations
  • Eye injury
  • Migraine/headache
  • Mild chest pain (no history of cardiac issues)
  • Minor head injury
  • Minor motor vehicle accident
  • Minor work comp
  • Nose bleeds
    *This list is not all inclusive

These visits DO NOT include the following:

  • History and physicals
  • Preventative care
  • Medication refills
  • Hospital/ED follow ups
  • Chronic symptoms
  • Quick care menu items

Same day appointments can be scheduled from 9 a.m. to 4:30 p.m., Monday through Friday.

Our Emergency Department (ED) is staffed by registered nurses (RN), advanced practice clinicians (ACP), and physicians who work together to assess, diagnose and treat emergent medical conditions. Lakewood is equipped with ambulances to transport patients to the hospital, and a helipad to transport patients elsewhere in emergency situations. We provide medical/trauma care, crisis intervention, and client education in a caring and confidential environment.

Non-emergent medical needs should NOT be seen in the Emergency Department. Quick Care, same day, primary care and Click Care options are available for non-emergent needs.

Lakewood’s Click Care is an online diagnosis and treatment service that connects patients with Lakewood providers. Start an online visit 24 hours a day, 7 days a week, and a provider will respond between 7 a.m. and 7 p.m., seven days a week.

Patients receive prompt care for a variety of common medical conditions such as:

  • Acne
  • Allergic rhinitis
  • Bladder infection/UTI
  • Burns
  • Cold/canker sore
  • Conjunctivitis
  • Diaper rash
  • Ear pain
  • Ingrown toenail
  • Lice
  • Motion sickness
  • Oral sores
  • Pink eye
