If you want to sign up without an activation code, go to mychart.ochin.org/mychartls/ or scan the QR code and use your name, date of birth and social security number to register.

Proxy access allows someone to log in to their personal MyChart account and connect to information regarding another person. The quickest way to set this is up is to have the person who wants to share their record log in to their MyChart account, select Sharing Hub from the main Menu and follow the prompts to send an invitation. Another option is to complete a proxy consent form and return it to Lakewood’s HIM department.
Anyone age 12 and older is eligible to sign up for a MyChart account. Proxy access can be granted to anyone age 18 and older.
Lakewood MyChart accounts created before August 11, 2023 have been archived and set to read-only, but can be accessed through this link: MyChart archived accounts.
They can also be found on MyChart.org. Search for Lakewood Health System and look for this logo:
Unfortunately, not all data/information will flow from your previous MyChart account to the new, but below is what you can expect to find once the two accounts are linked:
- Problems, allergies, medications and immunizations
- Anything a provider manually pushes out from Epic
To link your archived and new accounts (and any other Epic MyChart accounts you may have through other organizations), log in to your MyChart account, select Link My Accounts from the main Menu and search for Lakewood Health System.
