Make an online donation
Quickly and securely donate online to support programs, equipment, and patient care that go beyond what insurance covers.
Honor & memorial gifts
Celebrate a special occasion, wish someone a speedy recovery, or express condolences. All donations are acknowledged to the individual or family you’re honoring.
Estate planning
Include Lakewood in your will, trust, or estate plans to provide long-term support. Gifts can be specified as a percentage or a fixed amount, ensuring your generosity continues to make an impact.

Ways To Request Support
At Lakewood, our vision is to empower health and well-being together. That includes helping other organizations that support this community.
Community benefit grant program
$1,000 to $5,000 to support nonprofit projects that promote access to resources and care, improve quality of life, or enhance the built environments.
Charitable contribution
A monetary gift to nonprofits and other organizations whose mission and vision mirror those of Lakewood.
Sponsorship
Financial support of a community event or fundraiser in exchange for Lakewood advertising and/or logo placement.
In-kind donation
Donation of door prizes and/or give-away items to support a community event or fundraiser.
Foundation FAQs
Who is eligible to apply for community benefit grants?
To be eligible, you must:
- Be designated as a tax-exempt organization under IRS Code Section 501(c)(3), or have a fiscal sponsor who is tax-exempt
- Address at least one of the focus areas in our 2023-25 Community Health Implementation Strategy Plan (promoting access to resources and care, improving quality of life, or enhancing built environments)
- Serve residents in Morrison, Todd, or Wadena counties, or in Lakewood's key service area communities
What are Lakewood's key service area communities?
Our key service area communities include Browerville, Eagle Bend, Motley, Pillager, and Staples.
What types of projects will not be funded?
Grants will not be made for:
- Religious activities
- Political campaigns
- Memorials or endowments
- For-profit organizations
- Individual or family benefit
- Registration fees to charitable races and conferences, or travel expenses (ordinarily)
- Proposals that have already occurred
Additionally, if you're applying as a national organization, your application must focus on local or regional efforts.
When can I apply for a grant?
Grant requests are accepted on an ongoing basis throughout the calendar year. It is recommended that requests be submitted at least 60 days prior to needing the funds.
