What information do I need to provide when requesting medical records online?
Our online medical records request will ask for a photo of your driver's license to verify your identity. You can take the photo with a webcam or smartphone. This process works best in Chrome, Safari, and Firefox browsers.

Our team members obtain patient consent, certify medical records, and determine what information can be released according to privacy regulations. They can assist with any request for information, whether for personal use, transfer of care, legal matters, or insurance purposes.
How do I set up MyChart access or authorize a proxy?
Authorization and proxy forms for MyChart accounts can be found in our Documents Library. These forms allow Lakewood to share your personal health information with you, as well as a proxy, via the MyChart platform. Or give us a call.
How can non-Lakewood providers request affiliation letters?
Healthcare providers and organizations outside of Lakewood can request access to autogenerated provider affiliation letters from Lakewood.
What services does the Vital Statistics team provide?
Our Vital Statistics team gathers information and processes birth certificates with the state of Minnesota for babies born at Lakewood. We also offer to apply for your baby's social security card and send birth announcements to area newspapers. Additionally, our team works with medical providers to process death certificates and cremation approvals for your loved ones.
