Community Needs Health Assessment

Lakewood Health System's 2025 Community Health Needs Assessment identifies priority health needs in our community, as required every three years by the Affordable Care Act.

Our 2026–2028 priorities are:

  • Elevate community well-being
  • Ensure access to resources and care
  • Foster vibrant communities

Community Benefit Grant Program

One way we support healthier communities is through our Community Benefit Grant Program.

Lakewood awards funding to nonprofit organizations whose projects align with our Community Health priorities and help address local health needs. By supporting community-led solutions, we can leverage resources, strengthen partnerships, and create a greater impact across the region.

Grant requests are accepted throughout the year, with awards typically ranging from $1,000 to $2,500 and a maximum award of $5,000.

Healthy Connections


Todd-Wadena Healthy Connections was established in 1993 as a health coalition made of public and private health agencies serving the communities within Todd and Wadena counties.

Two work groups, Maternal Child Health and Health Educator, implement project activities at the community level, and meet the goals of the coalition board.

Mission

To provide a forum for communication and collaboration to improve the health of people in those communities.

Goal

To collaborate on building healthy communities Community Health Assessment, including coordination of activities to improve population health.

Community Health

Healthy communities don't happen by chance. Through our Community Health team, Lakewood partners with schools, public health agencies, local organizations and community leaders to improve the health and well-being of the people we serve.

Guided by our Community Health Needs Assessment, we develop programs and initiatives that address local needs, reduce barriers to care, and create opportunities for healthier lives.

Food Access Programs

Access to nutritious food is essential to good health. Lakewood offers several programs designed to improve food security and increase access to healthy, affordable food throughout our communities.
